Canada to Require Upfront Medical Exams for Express Entry Permanent Residence Applications Effective from 21 August 2025.
Immigration, Refugees and Citizenship Canada (IRCC) has announced a major change to the Express Entry application process.Starting 21 August 2025, all applicants for permanent residence through Express Entry will be required to complete their medical exam before submitting their application.
What’s Changing?
Currently, Express Entry applicants submit their complete PR application first, and then wait for IRCC to send instructions for completing the immigration medical exam.
From 21 August 2025, this process will change applicants must book, complete, and submit their medical exam results before applying.
Who Will Be Affected?
Affected: All Express Entry PR applications submitted on or after 21 August 2025.
Not Affected:
Permanent residence applications outside the Express Entry system.
Medical Admissibility Requirements:
All applicants and their family members, whether they are accompanying them to Canada or not must be medically admissible.
Medical inadmissibility can occur if:
A medical condition is likely to pose a danger to public health or safety; or
A medical condition is expected to cause “excessive demand” on Canadian health or social services.
For cost considerations, “excessive demand” is defined as exceeding three times the national average per capita healthcare cost, currently set at $27,162 per year.
Tips:Applicants with chronic health conditions such as diabetes can reduce their risk of refusal by providing evidence that their condition is well-managed, stable, and under regular medical supervision.
What You Should Do If Applying After 21 August 2025?
Book your medical exam early with a panel physician approved by IRCC.
Ensure your results are valid at the time of application submission.
Keep copies of your medical records and test results for reference.
This change is part of IRCC’s effort to streamline processing and reduce delays by ensuring medical admissibility is assessed at the start of the application process.
